Heart Failure / CHF Program
AHS utilizes registered nurses that work closely with patients and their family members and provide them education on numersous aspects and treatments of Heart Failure. Our care programs are designed to aid patients with health issues in their hearts, veins and arteries. Our nurses ensure that patients understand all forms of their disease process and treatment which include:
- Diet
- Fluid restriction
- Medications
- Smoking cessation
- Activities permitted
- Recognition of signs and symptons
- Weight management for early detection
AHS CHF Program concentrates on health guidance and teaching the patient to improve their overall health. Our supportive nurses provide nutritional counseling and treat patients from every aspect of their health. They provide in-home rehab and CHF Program that includes cardiac monitoring through telemedicine that helps our patients and their families have peace of mind.
Geriatric Mental Health
AHS utilizes Medicare-certified psychiatric nurses that perform psychiatric home visits, assessments, interventions and treratments. Our nurses are well trained to support the patient's medications with psychiatric disorders.
